TRAZAR Community Care is a heart-centered home care organization serving individuals and families across Pennsylvania and Minnesota.

Our purpose is simple: to provide compassionate, culturally sensitive care that helps seniors and adults with disabilities live safely, independently, and with dignity in the comfort of their homes.

 

 

 

Our Story

 

TRAZAR was founded from a genuine understanding of the challenges families face when a loved one needs care. We saw families struggling to navigate services, clients waiting for support, and caregivers who were not always matched with the right individuals.

We wanted to become part of the solution.

Today, TRAZAR serves families through two organizations:

Pennsylvania | TRAZAR Community Care Inc.
Providing non-medical home care for eligible Medicaid waiver and private-pay clients in Delaware County, Philadelphia, Chester County, and Montgomery County.

Minnesota | TRAZAR Community Care 2 LLC
Providing skilled and non-skilled home care services to individuals and families in Minnesota.
